Despite investing enormous time, effort and money into implementing technically advanced and innovative barrier elements in views of preventing hydrocarbon leaks on offshore installations, the statistics still stands far from the target zero. Yet, the efforts towards minimizing the number or leaks can never be ceased due to the catastrophic nature of its potential consequences to human, environment, asset or reputation. In Norway, there have been 110 hydrocarbon leaks between 2008 and 2017, some of which with vast potential for fire and explosion. It has been observed that human operators & maintenance personnel have played a vital role in some of the most serious near misses or accidents in the past, in negatively escalating a chain of events into a ripened disaster. The study has illustrated that 69% of the causes of the accidents emerged from human and organizational factors. The study also pointed out the focus that has been missing on some minor aspects within human and organizational regime in the industry.
